A health risk assessment is included with the wellness visit, which is a questionnaire designed to help your healthcare provider develop or update your personalized prevention plan.

An annual wellness visit includes, but isn’t limited to
- Assessment for detection of cognitive impairments, such as dementia or depression
- Review of medical and family history; including a review of current providers and/or medications
- Update medical records and a discussion regarding medical services you may need, like mammograms, colonoscopies, diabetic eye exams, and/or bone density screenings
- Review of risk factors and corresponding treatment options available to you
